WIP 124 - Improving Resident Comfort Level in the PICU through Implementation of a Pediatric Critical Care Core Curriculum Combined with Just in Time Learning

Background: Pediatric residents undergo extensive training but have limited time working in an ICU setting. This leads to a high variability of exposure to pathology and medical conditions, depending on the season of PICU rotation. QI studies from many institutions have identified that pediatric residents are dissatisfied with their level of critical care knowledge. This has led to QI projects to standardize curriculums and create learning interventions. Objective: The objective of our QI project is to increase resident comfort level in the PICU and satisfaction with their critical care education. Our approach is to revise the PICU curriculum and implement core lectures and just in time training tools. The smart aim is to increase resident comfort level in the 4 lowest rated topics by 20% and all 15 topics by 10% over 6 months. Design/Methods: The IRB waived approval for this study. Baseline data was collected from 2nd and 3rd year pediatric residents using anonymous RedCap surveys that queried their current level of satisfaction and comfort in caring for children with critical illness. A Likert scale measured 15 critical care topics: shock, respiratory failure, mechanical ventilation, bronchiolitis, status asthmaticus, traumatic brain injury, status epilepticus, sedation, acute kidney injury, electrolyte derangements, endocrine and hematologic emergencies, acute liver failure, procedures/monitoring techniques, and brain death testing/criteria. Survey response was 100%. PDSA cycle 1 is in progress and is planned from Oct-Dec 2023 with the initial intervention of weekly lectures covering the 4 lowest rated topics. Work hour log monitoring will act as a balancing measure to ensure there are no duty hour violations due to lecture time. Further PDSA cycles will be conducted, including the use of just in time learning tools, until the goal is reached. Repeat surveys will be collected post PICU rotations. This is an observational time series study with multiple sequential interventions. Run charts will be used to analyze data.
