365 - Examining Patient-Centered Care Amongst the Hispanic Pediatric Population

Background: Previous studies have examined the effects ethnic disparities have on measures of patient-centered care (PCC) amongst the pediatric population. However, it is unclear which factors influence the experience of PCC within the Hispanic population.

Objective: Among US children ages 0-17 years, examine differences in PCC practices (i.e., shared decision making (SDM) and family centered care (FCC)) by ethnicity, and the factors impacting these measures between the non-Hispanic and Hispanic populations.

Design/Methods: Cross-sectional analysis of the 2020-21 National Survey of Children’s Health (NSCH), an annual household-based survey funded by the US Maternal Child Health Bureau, yielding nationally representative data on children aged 0–17 years old in the US (weighted N= 58396272.8).

Main variables of interest in this study were ethnicity and measures of frequency receiving PCC as determined by responses pertaining to FCC and SDM per NSCH.

Covariates included sex, age category, born in US, parental nativity, primary household language, highest level of adult education, poverty level, region, insurance type, and having a provider families considered their “personal” doctor.

Statistical analyses were conducted using R to compare differences in PCC measures among those with at least one visit in the past year, stratified by ethnicity. Pearson chi-square was computed to assess for relationships between PCC measures both between ethnicity and selected covariates within the Hispanic population. Survey weighted logistic regression models were constructed to examine strength of relationships amongst this data.

Results: Among those surveyed, 25.7% identified as Hispanic. Of those, only 68.8% received well-child checks from 2020-21, versus 78.7% for non-Hispanics. For Hispanic children who received a well-child check, if English was not their primary household language they were 32% less likely to report FCC (95% CI: 0.52, 0.89) and 47% less likely to report SDM (95% CI: 0.33, 0.87). If they did not endorse having a personal doctor they were 56% less likely to report FCC (95% CI: 0.34, 0.57) and 55% less likely to report SDM (95% CI: 0.28, 0.74).

Conclusion(s): The Hispanic population presents the lowest rates of PCC measures amongst the pediatric population when considering race/ethnicity. Examination within this group shows specific factors (e.g., household language) that even further impact their experiences. Awareness of these influences and studying methods of mitigating the impact they have on PCC is imperative towards promoting satisfactory care within this population.
