Description: Formal training in Patient Safety (PS) is now required by ACGME in both residency, and fellowship training programs, as well as for maintenance of certification for practicing pediatricians. Solutions for Patient Safety (SPS), a collaboration of 140+ children’s hospitals, working together to improve PS, lists building a culture of safety as one of the top priorities towards improving patient outcomes. The concept of Just Culture - including classification of medical error types and establishing appropriate and just methods for responding to errors is critical to building a culture of safety. We will train attendees in practical, validated methods for building a Just Culture in their institution or training program. Attendees will be provided with relatable cases and hands-on practice classifying behaviors into “Human Error” “At-Risk” or “Reckless.” Through individual polls and small group interactive tasks, they will then apply the console-coach-discipline method appropriate to their cause analysis. Attendees will complete the session being able to apply a Just Culture algorithm (The Reason & Hobbs Culpability Decision Tree ) to various scenarios by choosing appropriate responses to errors using this best practice method.
