Description: This session involves as PRO/CON debate on 2 topics:
1) Congenital syphilis: benzathine penicillin is OK for the normal Infant
2) Neonatal herpes simplex virus (HSV) infection: asymptomatic newborns of mothers with active genital herpes infection: is a lumbar puncture and acyclovir needed? (Speakers David Kimberlin and "To be Announced")
Both of these topics are much discussed in clinical practice. The use of benzathine penicillin for infants born to mothers with untreated or inadequately treated syphilis and when the infant evaluation is normal has a long history of effectiveness, yet this therapy is not frequently utilized as many clinicians treat for 10 days of intravenous penicillin therapy. This results in mother-infant separation, NICU admission, and prolonged hospital stay. With respect to the newborn exposed to active "primary" maternal genital HSV lesions, the recommendation has been to evaluate the infant fully, including a lumbar puncture. Even when the evaluation is negative, a prolonged course of acyclovir is recommended, which means NICU admission, maternal-infant separation, and increased hospital costs.
