Workshop
Description: Many pediatric hospital systems are implementing health related social needs (HRSN) screening in response to The Joint Commission requirements and Center for Medicare and Medicaid’s quality measures. A critical but challenging component of this process is responding to identified needs in a patient-centered manner that also incorporates community-based organizations as partners. This workshop will prepare participants to lead their institutions in developing or expanding upon a healing-centered approach to addressing social needs within their institutional and community context.
The session will cover three different approaches to resource connection: 1) community resource referral platforms (ie Find help), 2) resource co-location, and 3) fostering community partnerships. Each strategy will be introduced in a large group didactic session discussing the challenges, benefits, contextual considerations, and potential evaluation methods for the resource provision approach. Strategies for incorporating the voice of the patient and community organization partners will be emphasized.
Participants will then have the opportunity to attend two different small group breakout sessions on the strategies of their choice. In small groups, participants will be able to share successes and challenges at their institution and will complete a program development tool with guidance from content area experts. At the end of the session, each participant will have a completed project development worksheet with actionable items for implementing/expanding and evaluating a social resource connection program at their institution.
