Description: Despite decades of research, controversy about the optimal use of systemic corticosteroids in neonates remains. Recent trends suggest growing use of hydrocortisone (HCTZ) among preterm infants. However, somewhat conflicting results on the short and longer-term benefits of HCTZ
has led to uncertainty regarding patient selection and optimal timing for initiation. While recent well-designed multi-center, randomized controlled trials on HCTZ have been informative, fundamental questions on safety and effectiveness remain unanswered. Moreover, renewed interest in the potential value of dexamethasone as an alternative to HCTZ warrants careful consideration. At present, healthcare professionals lack equity-focused, evidence-based data to guide their clinical management.
