Description: Over the last several years, many health systems have designed and implemented new social needs screening and referral programs. In 2024, as new regulations incentivizing or mandating universal social determinants of health (SDOH) screening are put into place, health care providers will need to adapt quickly and balance data gathering and reporting requirements with patients’ and families’ needs and preferences.
This highly interactive session led by a multidisciplinary team will start by reviewing current social needs screening regulatory requirements, including those of Centers for Medicare and Medicaid Services (CMS), Joint Commission, and National Committee for Quality Assurance (NCQA). We will then discuss implementation strategies, including how to incorporate patient and family perspectives and preferences, and provide effective resource referrals. We will explore technology-based strategies for screening, referral, and data management, including EHR-embedded screening tools, patient-facing resource maps, and approaches to data management and reporting. Finally, we will highlight examples of existing social needs screening and referral programs, with a focus on efforts to incorporate technology-based solutions, prioritize patient and caregiver preferences, and track and report program impact on patients and families.
