Description: Hospital-based neonatal and pediatric services in the United States have been both centralizing and decentralizing over the past 20 years, depending on the patient population and different geographic areas. Available information on trends have been most robust for neonatal hospitalizations, but are also emerging for pediatric hospitalizations.
This state-of-the-art plenary begins with a historical overview of the above five most common hospital-based services pertaining to newborns and youth in the United States—maternity/general care nurseries; neonatal intensive care units (NICUs); pediatric emergency care; general pediatric inpatient units, and pediatric intensive care units (PICUs). It will then move into recent national trends within the United States and, where data are available, examines how the service accessibility relates to utilization and health outcomes.
This plenary poses and examines four key ideas:
A. Whether hospital-based neonatal and pediatric services in the United States could be considered competitive marketplaces, i.e., have enough providers to compete on service options and outcomes?
B. Whether the organization of hospital-based neonatal services is fundamentally different from or similar to the organization of hospital-based pediatric services?
C. How the organization of hospital-based neonatal and pediatric services is related to subsequent newborn and child health outcomes?
D. What research or policy changes are needed to optimize the outcomes of hospitalized newborns and children during this time of rapid change in the organization of hospital-based neonatal and pediatric services?
