WIP 79 - Assessing Impact of Socioeconomic Determinants of Health on Access to a Comprehensive Fetal Care Program at a Tertiary Pediatric Center

Background: The Comprehensive Fetal Care Program (CFCP) at a tertiary pediatric center aims to optimize multidisciplinary care for the mother and fetus with prenatally diagnosed congenital anomalies. Care coordination by the CFCP aims to improve communication among the care team, allows for optimal delivery planning and ensures expectant parents gain crucial information about their child’s diagnosis and expected clinical course. Multidisciplinary care is provided by maternal fetal medicine and all relevant pediatric surgical and medical specialties. Socioeconomic determinants of health affect access to specialized, expert prenatal care in high-risk pregnancies.

Objective: This study investigates the impact of socioeconomic determinants of health on timeliness of referral to the CFCP and evaluates how socioeconomic determinants affect perinatal outcome.

Design/Methods: Retrospective chart review on maternal/neonatal patients enrolled in the CFCP at a tertiary pediatric center from January 2020 to December 2021. Socioeconomic status indicators included: race/ethnicity, primary language, insurance status, marital status, median income and distance to hospital based on ZIP code. Additional variables: gravida, parity, maternal age, type of anomaly, gestational age at birth. Outcomes measured: gestational age at referral, latency to specialty consultation, NICU admission rate and fetal/infant mortality.

Over 200 patients are included. The data will be analyzed through linear regression models, logistic regression models, chi-squared analyses, and analyses of variance with p value < 0.05.

Study approved by the Institutional Review Board. Data collection will be completed by January 1, 2024. Data quality assessment will be performed continuously and until January 15, 2024. Statistical analyses will be completed by February 15, 2024.
